​Patients with out of network plans can be provided with a superbill to self submit to your insurance for reimbursement​

New patients must schedule a New Patient Appointment before booking any other appointment types.

We ask all new patients also book a 45-minute follow-up about 3 weeks after their first visit to ensure we can follow up in a timely manner.

Your appointment is not considered booked until you receive an appointment confirmation AND you have completed all the questionnaires in the CHARM patient portal, including saving a card on file.
